About Client

One of the largest care management organizations serving patients in the New York area embarked on a journey to provide a Population Healthcare Management solution. The vision of this program is to provide better healthcare for populations at a lower cost. To make that vision a reality, partnership is needed with a technology service provider who can help them innovate and deliver at a fast pace.

  • Implemented the Population Health Analytics platform using IBM Unified Data Model for Healthcare;
  • Custom Patient MDM and Provider MDM solutions; Analytics using Cognos, SAS and other products
  • Integrated Claims, Clinical, Care Management & SDOH data from EPIC, CCMS, QNXT and 12 other health plans
  • Built a risk stratification framework and predictive analytics initiatives including Patient Insights - to provide a 360-degree view of patients
